BELLMONT HS ATHLETICS

The Bellmont High School Athletic Department is very excited to announce that we have partnered with FinalForms, an online forms and data management service. FinalForms allows you to complete and sign athletic forms for your student(s). FinalForms also pre-populates information wherever possible, for each of your students and any cross-form redundancies (medical info, etc.). Both are time savers. You may renew your data at any time to verify it is current. A parent and the athlete will be required to sign forms one time per year and after any update. The IHSAA physical form is one of those forms that needs to be updated annually anytime after April 1 for the following school-years’ athletic participation. For the 2023-2024 schoolyear, incoming freshman through current juniors will need to have all forms completed before summer participation starts. For most sports, that date is May 30th. Bellmont HS has partnered with Adams Memorial Hospital for Wellness Exams/Sports Physicals where athletes can have their IHSAA Physical forms completed. Please note that Saturday appointments are available for the spring/summer of 2023 at Adams Medical Group Physician Offices. Information for scheduling at AMH, the IHSAA Physical Form, as well as the FinalForms Parent Playbook can either be printed below or can be picked up from the BHS main office. While working through the FinalForms process, If you require support, scroll to the page bottom and click “Use Support”. Quick access to the FinalForms website can be done by clicking the FinalForms icon in the left column of the www.bellmontbraves.com Bellmont Athletics Homepage and follow the prompts to login/create your student(s) ID and sign your forms.
